The Check-In
A colleague asked me this week how I manage stress during fellowship.
I told her I don't manage stress. I manage recovery.
The distinction matters. My Mind score fluctuates between a 3 and a 6 depending on the week. Some weeks are heavy. Long cases, high acuity, the emotional weight of operating on children's faces. I don't try to prevent the dip. The dip is real. It reflects something true about the demands of the work.
What I track is how quickly I return to baseline.
Three weeks ago, after a hard stretch, my Mind stayed at a 3 for four consecutive days. That was a signal. Not that I was stressed, but that my system had lost its ability to reset. The stress wasn't the problem. The stickiness was.
This week, after an equally demanding stretch, Mind hit a 3 on Monday and was back to a 5 by Wednesday. Same intensity. Faster recovery. The difference was that I'd been consistent with sleep, I'd been moving, and I'd had two real conversations with friends in between.
The Alignment Pulse doesn't measure whether you're stressed. It measures whether your system can recover from stress. That's a different question entirely. And it's the more important one.
The Insight
Most people who come to PT's door won't be sick. They won't have a diagnosis. Their labs will be normal. Their doctor will tell them they're fine.
They won't be fine. They'll be stuck.
The clinical concept behind this is allostatic load, first described by Bruce McEwen at Rockefeller University. Allostasis is the body's process of achieving stability through change. Your cortisol rises in the morning to wake you up. Your heart rate increases during exercise. Your immune system activates during infection. These are adaptive responses. The system shifts to meet the demand, then returns to baseline.
Allostatic load is what happens when the return doesn't come. When stress responses activate repeatedly without adequate recovery, the system starts to wear. Not catastrophically. Subtly. Sleep becomes less restorative. Cognitive flexibility narrows. Emotional responses become more reactive and less proportional. The body still functions, but the margins shrink.
McEwen's research, published across decades in the Annals of the New York Academy of Sciences and elsewhere, demonstrated that allostatic load predicts cardiovascular disease, cognitive decline, and mortality, often years before traditional biomarkers show anything abnormal.
This is the population PT is built for. People who still function well but no longer reset well. They're not candidates for medication. They don't need a diagnosis. They need a measurement system that detects the erosion of their recovery capacity before it becomes a clinical problem.
That's what the Alignment Pulse does. Six domains. Every day. The score isn't the point. The pattern over time is. A single low Recovery score is a snapshot. Five consecutive low Recovery scores with declining Mind is a trajectory. And trajectory is where the clinical signal lives.

The Evidence
This week's study: Allostatic Load as a Marker of Cumulative Biological Risk
McEwen and Stellar, Annals of the New York Academy of Sciences, 1993. This foundational paper introduced the concept of allostatic load as a framework for understanding how chronic stress produces wear on biological systems.
The key insight: the body's stress response is not inherently harmful. Cortisol, adrenaline, and inflammatory mediators serve essential adaptive functions. The damage comes from overexposure, meaning repeated activation without adequate recovery. McEwen identified four patterns of dysregulation: repeated hits of novel stressors, failure to habituate to repeated stressors, prolonged stress response after the stressor ends, and inadequate stress response leading to compensatory hyperactivity in other systems.
Subsequent research validated allostatic load as a predictor of health outcomes independent of individual biomarkers. A composite index of stress-related biological markers predicted cardiovascular events, cognitive decline, and all-cause mortality.
The clinical implication for PT: measuring how someone feels today is less valuable than measuring whether their system returns to baseline after stress. The Alignment Pulse is designed to detect that pattern through longitudinal tracking across six domains.

The Build

I've learned something about pace over the past month. There were stretches where I was posting daily, tracking hourly, iterating constantly. The output was good. But the pace wasn't sustainable alongside a surgical fellowship. And if I'm building a product about sustainable health practices, the way I build it should reflect that.
